Fat Transfer vs Breast Implants – or both?

Hybrid Methods of Breast Augmentation may provide enhanced results for certain patients.

Your anatomy may limit what can be done with a surgical augmentation approach. Some patients may get better outcomes when traditional implant-based augmentation is combined with fat transfer to breasts.

To get both breast implants and liposuction/fat transfer involves MORE surgery procedures and additional costs. Plus all invasive surgery has risks and results will vary from patient to patient.

Hybrid Breast Augmentation and Fat Transfer to Breasts – Costs, Pros and Cons

Combining FAT transfer to breasts with implant-based breast augmentation methods is known as a HYBRID Breast Augmentation procedure.

PROS: Why try a fat-transfer with implants HYBRID breast augmentation approach?

  • Attaining a fuller shape and/or noticeable enlargement of the breasts typically requires an implant-based augmentation approach – with or without additional fat transfer.
  • Fat transfer to breasts MAY help select patients get better breast shaping outcomes OR cleavage enhancement results.
  • In thin patients or body builders with adequate donor tissues for fat transfer, this method may help soften the appearance of implant-augmented breasts; providing a more natural looking appearance.

CONS of Fat Transfer

  • On its own, fat transfer can achieve only small size increases or minor shaping enhancements.
  • Not all patients have adequate sites of “donor fat” (and no, you can’t share fat with friends!)
  • Results can vary significantly FROM patient to patient and even WITHIN patients over time, when body fat changes, so, too, might your breast size or shape.
  • Fat transfer COSTS are a consideration for some patients as it required additional procedures, often multiple fat transfer sessions for some patients.
  • All invasive surgery has risks and all plastic surgery results vary from patient to patient.

Be sure you ask your Surgeon for a list during a consultation and be sure you understand the costs, risks, healing processes and recovery time before you consent.

FAT TRANSFER: Liposuction, Distribution and Fat Grafting

  • The procedure of fat transfer (autologous fat transfer) uses tumescent liposuction to attain fat – this is often called ‘harvesting fat’ from a donor site of the body (hips, buttocks, waist).
  • The fat is then filtered and processed.
  • The fat is then injected into the desired area via a cannulae.

Fat grafting Benefits and Concerns:

There are benefits to the procedure as an adjunct to a standard breast augmentation using silicone or saline breast implants.

However, fat grafting has some limitation in terms of:

  • number of procedures required
  • amount of volume achievable
  • uneven skin consistency or globules/bumps or lumps after transfer
  • lack of uptake or stability

Is fat grafting to breasts best performed as an stand-alone procedure – or as an adjunct to Breast Augmentation?

The answer is that it depends on numerous factors, including but not limited to your anatomy, your preferences, your skin, your natural breast tissue, and your donor sites.

Results also depends on the areas being treated and the Surgeon’s experience with fat transfer.

Why does fat transfer cost so much?

 

  • fat grafting involves two (2) operations for every procedure, and there are many steps your Surgeon need to take, including:
    • preparing you for surgery
    • removing fat from the donor site using liposuction methods
    • carefully processing the fat to reduce certain types of cells to reduce risks
    • reinserting the processed adipose cells/tissues through fat transfer injections (grafting)

Advantages of Fat Grafting vs Breast implants

  • If used alone without implants, there tends to be less scarring in the breast area (injections versus incisions) – but you will have scars, and scars in the liposuction treated areas
  • Breast Implants are not required – the surgery uses your own tissues
  • Your breasts will feel and look more natural, including during dynamic movement
  • Less weight on skin than an implant (but may not suit skin ptosis)
  • Fat removal lipo-contouring processes may help to contour fat donor sites on your body
  • Some researchers believe it also improves skin appearance due to cellular renewal processes

Disadvantages of Fat Grafting vs Breast Implants

  • A much less noticeable size change than using breast implants
  • Less stable results than predictable breast prostheses which have
    • known sizes, more stable shaping consistencies
    • precision placement techniques
  • It is difficult to assess how much fat to remove and to inject at each session, based on inconsistent uptake
  • Breast Asymmetry may also be an issue (differences in glandular tissue composition of breasts)
  • Low BMI patients may lack adequate donor sites
  • Weight and BMI changes may impact results and uptake is unpredictable
  • Fat grafting to breasts provides a much less noticeable change than using breast prosthesis (implants)
  • Implants may be a better option if you want a noticeable increase
  • Multiple procedures are required over a shorter period of time in order to gain enough volume
  • Risk of infections and other complication risks at donor sites
  • Potential uneven uptake or bumps or lumps at injection sites

An ALTERNATIVE: Combining FAT Grafting with Implants for a HYBRID Breast Augmentation Procedure

  • There may be BENEFITS from COMBINING both fat grafting to the breasts with quality BREAST IMPLANTS from Mentor or Motiva.
  • This is called a HYBRID breast augmentation, meaning silicone-filled or saline-filled breast implants are combined with fat transfer to the breast(s).

For example, hybrid breast augmentation may allow for:

  • extra shaping of the cleavage area OR
  • help to cover edges of a breast implant in a thin person with enough donor fat tissues.

This procedure may NOT be suitable for all candidates or desired breast size increase goals in relation to cosmetic breast surgery.

Is Fat Grafting going to be the future mainstay of Breast Augmentation?

  • Fat to breasts via liposuction and tissue grafting MAY eventually have the potential to replace the use of silicone implants.
  • But we are nowhere near that yet.
  • The procedure often requires multiple surgeries to have an effect AND can be costly and time-consuming.
